Cardiac Rehabilitation
This program is for patients who have had a myocardial infarction, stable angina, or bypass surgery, angioplasty and Stent placement. This exercise/education program helps the patient return to everyday life by managing their disease process through lifestyle and behavior change. Participation is by physician referral and is usually covered by Medicare and private insurance carriers. This program is certified by the American Association of Cardiovascular and Pulmonary Rehabilitation.
Congestive Heart Failure Exercise and Education Program
This program is for patients who have been diagnosed with Congestive Heart Failure (CHF). Our 12- week supervised exercise and education treatment plan can help the CHF patient along the pathway to a more fulfilling and enjoyable life. Some of the program components include: daily weighing, sodium reduction, home activity and many others.
Pulmonary Health and Rehabilitation
Pulmonary Rehabilitation assists patients with lung disease achieve their optimal capacity to carry out daily activities through individually tailored programs. This program is especially beneficial for people who suffer from asthma, emphysema, chronic bronchitis and interstitial lung disease and many others. This program includes an exercise and education component.
Peripheral Arterial Disease Exercise and Education Program
This program is for patients that have been diagnosed with Peripheral Arterial Disease (PAD). The program consists of supervised exercise along with educational classes. Our treatment program is designed to diminish symptoms, delay the progression of the disease and improve the quality of life of the patients through changes in diet, exercise, and lifestyle habits.

Smoking Cessation
*FREE* 4-week smoking cessation course sponsored by Heartworks and the American Cancer Society. This program includes four 1-hour sessions that emphasize smoking cessation as a “two-part process” (stopping and staying stopped). Class setting involves group discussion and social support for individualized situation. This program also offers guidance in choosing which method of quitting is best for you.
